Please bring the following items with you for your appointment:

• Appropriate referral from your primary physician (If required by insurance)
• All health insurance cards
• Emergency contact name and phone number
• All outside x-rays (CT scan, PET scan, MRI, Ultrasound, Plain films, and reports), lab reports, pathology reports, pathology slides, and operative reports from other institutions unless other arrangements have been made
• All medical records from your current or referring physician unless other arrangements have been made
• All medications or a list of all medications you are currently taking and how often
• Your appointment confirmation letter from the Arkansas Cancer Research Center

You may be responsible for a $20 fee at the time of service If your insurance requires a co-payment, you will need to pay it at the time of service If you are unable to make your scheduled appointment, please call to notify the clinic as soon as possible. We will reschedule your appointment for a more convenient time.
