Preventive Visit Plan Includes:

- Comprehensive History and Physical Exam
-Chest X-ray
- Electrocardiogram
-Spirometry (Pulmonary Function Test)
- Audiogram (Computer-Directed Hearing Exam)
- Vision Screening
- Hemocult (Stool test for hidden blood)

Laboratory Testing Included:

Hematology:
 
-Hematocrit and Hemoglobin
-White Blood Count with differential
-Platelet Count
-Sedimentation Rate

Blood Chemistry:  
-Glucose
-Blood Urea Nitrogen and Creatinine
-Sodium, Potassium, and Chloride
-Calcium, Phosphorus, and Magnesium
-Iron, Ferritin
-Uric Acid, Protein, and Carbon Dioxide

Liver Function:  
GGT, GPT, COT, LDH, and Alkaline
Phosphates, Bilirubin

Lipid Panel:  Cholesterol (Total, HDL, LDL)
Triglycerides

 Thyroid Panel:  TSH, T4 and free T4 (Every 5 years)

 Urinalysis

 Prostate Specific Antigen (males)

 HIV Screening (optional)

Recommended Optional Testing:
- Treadmill Stress Test (every 3 – 5 years)
- Mammogram (every 2 years with past normal test)
- Pap Smear (Females, Annually)
- Colonoscopy (every 5 – 10 years with past normal exam)
- Optional Cardiac CT Scorning

- Other referrals or tests may be recommended following the exam.
