Registration for Annual Safety Training for All OFFSITE UAMS Employees Training

Completion of this form will register the user for training credit with Occupational Health & Safety for taking the course listed above.

Competency Test: To receive credit for this training, a score of 80% or better must be achieved to pass this safety course. The format for the test is multiple choice and True/False questions. Please complete the following registration information, then click on the "Begin Training" button to begin.

You must get 8 of the 10 points possible, in order to get credit for this training.
