Registration for Radiology Staff - Annual Radiation Safety Training Training

Completion of this form will register the user for training credit with Occupational Health & Safety for taking the course listed above.

Competency Test: To receive credit for this training, a score of 73% or better must be achieved to pass this safety course. The format for the test is multiple choice and True/False questions.
