The following three forms are used to request approval from the Radiation Safety Committee for use of radionuclides at UAMS. Form 1 and 2 are required for all applications. Form 3 is only necessary for animal use applications. The forms are in Microsoft Word 97 format. (There are NO macros in these documents.) If you are unable to use these documents in this format.
