Steps to follow before wearing a N95 Respirator

STEP 1: Complete the N95 Respirator Medical Evaluation Questionnaire.

  • Send the completed Questionnaire to Employee / Student Health by Fax 296-1230 or send to Mail Slotl #530-8.
  • Please wait 3 business days after faxing or mailing questionnaire to Employee/Student Health before coming to be fit tested.
  • Re-submission of the Medical Evaluation Questionnaire is required:
    • Every 2-5 years for those who are normally healthy depending on age at time of initial questionnaire submission:
      • every 2 years if age was 45 years and older
      • every 3 years if age was 35 - 44 years
      • every 5 years if age was 34 years and younger
    • Every year if an individual has an existing or develops a medical condition that requires yearly examinations.

STEP 2: Respiratory Training must be completed before being fit tested.

Training must be completed within the last 3 months before coming to be fit tested.

STEP 3: Respiratory fit tests are required for the use of any respirator with a tight-fitting face piece such as an N95 respirator.

Hours fit tests are performed
Monday       7:00 – 10:00 am
Wednesday  1:30 –  3:00 pm
Thursday     7:00 – 10:00 am

  • No appointment is needed.
  • Do not eat or drink anything except water at least 15 minutes before coming to be fit tested.
  • Males must be clean shaven to be fit tested. A mustache and goatee is allowed as long as it does not go under the chin to the neck.
  • Come to Central Building (old hospital ) G-154 to be fit tested. (1st door on left as you leave Old Central Sterile Supply and head towards Medical Records)

STEP 4: Annual fit tests are required to monitor changes in facial structure and ability to wear the respirator as well as annual training.
