Family Map Forms

 

Family Map Interview forms are free and available from the developers.  The forms below are for your review.  While we recommend training for staff before the use of the Family Map, contact the developers to discuss the implementation that is most appropriate for your setting.

Interview setting

Interviews in the family’s home:

Head Start Form (English)        

This form was developed for use by Head Start educators (often the classroom teacher) to be used during fall and spring home visits.  Use one per family per interview.

 

Interviews in classroom settings and not in the family’s home:         

Non-Head Start/Non-Home Visit Form (English)       

This form was developed for use by early childhood educators (often the classroom teacher) to be used during parent-teacher conferences typically conducted in the classroom or in another school setting. Use one per family per interview.

 

Additional forms needed for interviewing

General Response Card

This is a one page sheet used to prompt the parent with possible responses to each question.  The use of this form allows the parent to point to responses when conducting the interview in the presence of others and allows the interview to be conducted more quickly.  We offer it printed on heavy card stock with the TV/Movie rating on the back.  Each interviewer, regardless of setting, needs one of these cards.

 

TV/Movie Rating Response Card

This is a one page sheet used to prompt the parent with possible responses to questions regarding TV use in Section 2.  The use of this form allows the interview to be conducted more quickly.  It should be updated periodically with popular TV/Movie listings.  We offer it printed on heavy card stock with the General Response on the back.  Each interviewer, regardless of setting, needs one of these cards.

 

Observation Summary Card

This document summarizes the training materials regarding the observational items to be coded by the interviewer during the interview.  The interviewer should review this material before each home visits.  Refer to the appropriate Section documentation for more detail.  Each interviewer, regardless of setting, needs one of these cards although some items may not apply if the assessment is not conducted in the home.

 

Forms available in Spanish language

The following forms are a translation of the version above for interviewing Spanish speaking families.  We recommend using these instruments for the interview but recording responses on an English version unless all critical staff are bilingual. Use one per family per interview.
