The Employee Benefits Fair (Wed, Nov 17, 9-3:30, ground floor Ed II) is the kick-off open enrollment event.

How to Enroll/Change Coverage

Forms must be received in our office by 4:30 pm on Friday, 12/17/2010. You can print the forms listed below, fill them out, and mail to Human Resources/Benefits, box 564-1 (keep a copy for your records). Or you can fax to (501) 603-1318; be sure to keep the original and your fax confirmation as proof. FSA is the only paperless enrollment process – see details below.

You want to enroll in Medical as a new member, add eligible family members to your current coverage, or drop one or more family members (but you remain covered)

You are already enrolled in Medical but want to switch to the other plan option: Point of Service or Classic

Medical change of election Form

You want to enroll in Dental as a new member, or add eligible family members to your current coverage, or drop one or more family members (but you remain covered)

Drop Medical or Dental coverage

Call 686-5650 or pick up form in our office. We want to confirm coverage end date with you.

Change how Medical, Dental or Vision premiums are deducted from your paycheck: pre-tax premium conversion or regular after-tax

Visit our office if you do not have access to a computer and we can help you log-in and enroll.

Not sure what QualChoice will pay on a preventive screening procedure or other medical procedure you know you’ll have done in 2011? Call QualChoice at 219-5133 or 1-866-724-3570 so you can plan accordingly.
