· Year-end Benefit Options: summarizes the changes that take effect in January 2010 and your options to make changes.

· Flexible Spending Account: explains how an FSA works and why this may be a good option for you.

· Orientation Insurance Presentation: If it’s been a while since you went to orientation as a new employee, view this as a refresher.

· Benefit communications that have been mailed (or are in the process of being mailed):

· Summary of benefit changes and options for 1/1/2010, mailed to employees’ homes: Cover letter, Your Benefits for 2009 enclosure.

· · Medical and Dental open enrollment, mailed to employees’ campus mailboxes: cover letter, medical & dental rates, medical plans comparison, dental schedule of benefits, Medicare Part D creditable coverage letter.

How to Enroll/Change Coverage

Forms must be received in our office by 4:30 pm on Friday, 12/18/2009. You can print the forms listed below, fill them out, and mail to Human Resources/Benefits, box 564-1 (keep a copy for your records). Or you can fax to (501) 603-1318 (keep the original and your fax confirmation as proof). FSA is the only paperless enrollment process – see details below.

You want to enroll in Medical (special open enrollment) or add eligible family members to your current coverage

You are already enrolled in Medical but want to switch to the other plan option: Point of Service or Classic

You want to enroll in Dental (special open enrollment) or add eligible family members to your current coverage

Drop Medical or Dental coverage

Change how Medical and/or Dental premiums are deducted from your paycheck: pre-tax premium conversion or regular after-tax

Enroll in a 2010 health care or dependent care flexible spending account

Enrollment Instructions

Enroll online at https://mybenefits.conexis.com/.

PLEASE do not enroll in a health care FSA until you receive information about 2010 medical plan changes, so that you can plan accordingly.
