Vision Plan
Superior Vision Benefit Summary, Jan 2010 Who should sign up for Vision? While the plan is open to all benefit-eligible employees, it is really intended for those who wear glasses or contacts. The plan helps lower your out-of-pocket costs when you purchase frames, lenses or contacts. It does not cover medical problems associated with your eyes – those would be covered under our Medical plan. Superior Vision replaced Spectera as our vision plan carrier 4-1-2008. A second option called “Enhanced” plan was added 1-1-2010. You may choose between the Enhanced plan or the Basic plan. The difference between the two plans are the allowance amounts for frames and lenses. New employees may enroll in Vision at orientation. Open enrollment periods will be determined by Superior Vision and are not guaranteed to be held annually. Since you will likely only use the benefit once a year (the plan year is the calendar year), be sure to go to a provider in Superior Vision’s network to get your best benefit. UAMS Jones Eye Institute is in the network for U of A employees. Following are your premium costs, effective 1-1-2010:
COBRA Vision Rates - continuation of group health care insurance plans, at employees’ expense, after the employee leaves UAMS.
