Full-time employees are eligible for the leave benefits listed below. Additional information is included in the Staff Employee Handbook. Part-time employees may be eligible for leave on a prorated basis if you work at least 20 hours a week.

Residents and Housestaff do not accrue leave as regular faculty and staff and should contact their Housestaff Office for specific information.

Catastrophic Leave Physician Statement	Must be completed by the employee and their physician
Catastrophic Leave Bank Program Donor Form	Used to donate sick and vacation hours to the Catastrophic Leave Program
FMLA HIPAA Authorization Form	Must be completed by the employee
FMLA Certification of Health Care Provider	Must be completed by the employee's physician
Leave Request/Clocking Exceptions Daily	Used to request time off and to correct clocking exceptions
Clocking Exceptions Weekly	Used to request time off and to correct clocking exceptions
