The UA Medical Plan is self-insured and self-funded.    QualChoice is our third party administrator and processes your medical claims.  Caremark is our pharmacy benefit manager and processes your prescription claims.

 

UAMS Medical Plans

 

You have two medical plans to choose from:   Point of Service  or Classic Managed Care.     [If you live outside the State and QualChoice provider area, you have a third option called the “Alternative Point of Service” plan.  Contact Human Resources at (501) 686-5650 for further information.]     Both plans cover a wide range of traditional expenses such as doctors’ visits, surgical services, pregnancy, emergency room services, hospital stays, and diagnostic testing.   With a goal of assisting you in being healthy, the plans also provide coverage for items such as well baby check-ups, mental health counseling and prescription drugs.  

 

Point of Service Plan

 

The Point of Service plan is a “dual option” plan.   Your out-of-pocket costs are determined by whether you seek care from an in-network provider or an out-of-network provider.   You get your best benefit by seeing your PCP (primary care physician) or other in-network provider.   Seeing a doctor outside the network means you’d pay a higher share of the bill.  If you’re traveling out of state without access to network providers, you may lower your costs by going to a provider in the national Private Health Care Systems network, www.phcs.com.

 

Classic Plan

 

If you select the Classic plan you will enjoy a lower premium cost than the Point of Service plan.   However, benefits are only provided if you access care through your PCP or other in-network provider, similar to an HMO.    There are no benefits for out-of-network providers other than emergency care while traveling.  

 

New employees have a waiting period before coverage takes effect.  Once you enroll and complete the required forms, coverage takes effect the first of the following month.

 

Be sure to enroll within your first 31 days of employment.    Late enrollments are not allowed, nor does the University of Arkansas offer an annual open enrollment.   However, if you experience a qualified family event in the future, you have a 31-day window to make changes.

Following are the benefit forms that you can print, complete, and send to Human Resources.    Not all forms are listed here, as some are printed on multiple-copy paper and are only available through our Employee Services office, located in Room 1R/08 of the Barton Research Building.   If you are unable to visit Employee Services, please call (501) 686-5650 and ask for the form to be mailed to you.

QualChoice Claim Form Your health care provider should file the claim directly to QualChoice.  Use this form only if you had to pay your entire medical bill up front.
QualChoice Coordination of Benefits Questionnaire If you have other medical coverage, QualChoice may need you to complete this form.
QualChoice Disabled Dependent Form If you have an adult child who is disabled, Qualchoice may request this form. 
Caremark Claim Form Contact Caremark at 1-800-375-2596 before filling out this form, so they can contact your pharmacy and try to have your prescription drug claim reprocessed.
Student Status Verification Send to QualChoice each semester to verify your child age 19-24 is a full-time student.
Taxation of Premium Deduction Change the way your Medical premium is deducted (before-tax or after-tax)
