Life insurance provides a payment to family or other beneficiaries in the event of death. UAMS offers three group, fully-insured life plans
underwritten by Unum.

Basic Life Insurance

All benefit-eligible employees are automatically enrolled in Basic Life Insurance. There is no cost to you; this is a UAMS-paid benefit. It is a "term life" policy, the term being your UAMS employment. The benefit is 1 times your annualized salary, up to $50,000. This would be payable to your beneficiary in the event of your death.

Optional Life Insurance

Need more life insurance on yourself? You may purchase an additional 1, 2, 3 or 4 times your salary (up to a maximum of $500,000) by enrolling in Optional Life Insurance. Rates are based upon your age (see below).

Dependent Life Insurance

Need a policy on your family members? You may purchase life insurance on your spouse and dependent children by enrolling in Dependent Life Insurance. You are the automatic beneficiary in the event an eligible family member dies. You may choose $10,000, $15,000 or $20,000 coverage. Full amount is paid for spouse�s death, one-half for child�s death.

Optional Life and Dependent Life coverage is guaranteed-issued if you enroll within your first 31 days of eligibility. Late enrollment is subject to �evidence of insurability� (i.e. medical screening) and approval by the carrier.

Contact Human Resources at 686-5650 if a death occurs in your family. Please provide a death certificate and we will take care of processing the death claim for you.

Life Insurance Forms
Claim Form	To file a death claim. Human Resources will handle this for you. All we need from you is a copy of the death certificate
Conversion Form and Rates	Application and rates to convert life insurance to a private policy when you leave UAMS
Portability Form, Rates	Application and rates to "port" life insurance to a private policy when you leave UAMS. Applies only if you do not have a medical condition which has a material effect on life expectancy.
