Long Term Disability Insurance
Disability coverage provides you a benefit in the event a long-term injury or illness prevents you from working for more than 6 months. The benefit is 60 percent replacement of your salary, up to the maximum benefit. Our Long Term Disability (LTD) plans are group, fully-insured plans underwritten by Unum.
Basic LTD Insurance
All benefit-eligible employees (except FGP members, Housestaff and Medical Residents) are automatically enrolled in Basic LTD Insurance. There is no cost to you; this is a UAMS-paid benefit. Basic LTD provides a maximum monthly benefit of $1,000. This represents 60% replacement of your first $20,000 of salary.
Optional LTD Insurance
If your annual salary is over $20,000 you may enroll in Optional LTD Insurance. Optional LTD covers your first $100,000 of salary and therefore raises your maximum monthly benefit from $1,000 to $5,000. Optional LTD is a guaranteed-issued policy if you enroll within your first 31 days of eligibility. You are eligible to enroll in Optional LTD when you are first hired or when your annualized salary first exceeds $20,000. Late enrollment is subject to medical screening (i.e. “evidence of insurability) and approval by Unum.
FGP Members: You are automatically enrolled in a carve-out LTD plan called Basic FGP LTD Insurance, at no cost to you. This provides a maximum monthly benefit of $5,000 and covers your first $100,000 of salary. You must elect taxation of this benefit through Human Resources when you are first hired. If your salary exceeds $100,000 you have an additional carve-out plan called Expanded LTD that is administered through James D. Foss & Associates, 221-3700. You will need to enroll through Mr. Foss within your first 30 days. Generally, Expanded LTD premiums are paid on your behalf by the FGP and you will make an election through Mr. Foss on taxation of the premium. If you elect to pay taxes on the premium now, future benefits would be tax-free. Click here to learn more about your FGP benefits.
Housestaff and Medical Residents: You have a carve-out LTD plan that is administered through James D. Foss & Associates. Contact that office at 221-3700 within your first 30 days for your options and to take advantage of the guaranteed-issue window.

Not all forms are listed here, as some are printed on multiple-copy paper and are only available through our Employee Services office, located in Room 1R/08 of the Barton Research Building. If you are unable to visit Employee Services, please call (501) 686-5650 and ask for the form to be mailed to you.
