How to update your address with your benefit carriers

Thanks for keeping your home address updated.    

This is important, as UAMS occasionally mails information to your home.   For example, W-2s are mailed to home addresses.    Important benefits enrollment material is often sent to your home so that your family can also review it.  

When you update your address at UAMS, it may not necessarily flow to your benefit carriers.   We are working with our carriers to change this, so that they accept this data electronically.     But for now, you may need to personally notify some of your benefit carriers of your new address.

If you have the following benefits, please note the additional action you may need to take:

• Dental, Flexible Spending Accounts, TIAA-CREF and Fidelity retirement:    You don’t need to do anything.   These carriers accept our electronic file of current addresses and should update your information within a month.

• Medical:  Log in at www.qcark.com and change your address online.   Or call QualChoice at 219-5133.   QualChoice will notify MedImpact.

• Vision:   send an email to uofa@superiorvision.com.   Provide your old and new address, plus your ID # from your card (do not list your SSN).

• Home/Auto:   Call Liberty Mutual at 1-800-524-9400

• Critical Illness:  Call MetLife at 1-800-438-6388

• Long Term Care:   Call CNA at 1-877-777-7092
