The UA Dental Plan is self-insured and self-funded.
Delta Dental of
Arkansas is the third party administrator and processes your claims.
The
Dental plan is designed to assist you in maintaining good oral health. The
plan covers basic dental exams, restorative care, cleaning services and
preventive services. It also covers more intensive and specialty dental
needs including fluoride treatments, extractions, sealants, oral surgery,
crowns, bridges, spacers, and implants. This plan is a preventive plan and does not
provide orthodontia benefits.
There
is a waiting period before coverage takes effect. Once you enroll and
complete the required forms, coverage takes effect the first of the following
month.
Be
sure to enroll within your first 31 days of employment. Late enrollments are
not allowed, nor does the University of Arkansas offer an annual open
enrollment. However, if you experience a qualified family eventin the future, you have a 31-day window to
make changes.
Once
you are enrolled you can search for a dentist, benefits, and check your claim
status by logging in as a subscriber at
Delta Dental. However, do not use the
enrollment form found on their website. UA Enrollment forms are available
only in
Human Resources.

Dental Forms
Following are the benefit forms that
you can print, complete, and send to Human Resources. Not all
forms are listed here, as some are printed on multiple-copy paper
and are only available through our Employee Services office, located
in Room 1R/08 of the Barton Research Building. If you are unable
to visit Employee Services, please call (501) 686-5650 and ask for
the form to be mailed to you.
Once you complete your form, you can
drop it off at Employee Services. Or you can send it through
campus mail to Benefits, box # 564-1. Or you can fax it to UAMS
