Category Control Sheet
Preparation of the CATEGORY sheet:
If categories are not used, simply omit the CATEGORY sheet entirely. However,
if categories are used, then every item should be assigned into one and only one
of the 12 categories (11 categories when multiple choice items are combined with
a special score entry on the same test). If an item is left blank on the CATEGORY
sheet, it is assigned to category 1.
On the CATEGORY control sheet, the OTS course number must be filled in, the initials
must read "CAT”, and the ID must be 999999998. This may already be printed
if you requested pre-printed exam sheets.
Then, for each item, fill in its category number. Note that you have only
5 places to make a mark (ABCDE), but you may need to indicate up to 12 categories.
A system of multiple marks is therefore used.

Example: If you wish Item 45 to be in category 9 simply darken bubbles on item 45 with B and E.
