Please complete the attached Applicant’s Statement of Interest outlining your interest in pursuing recognition as a resource nurse in the Resource Nurse Recognition Program.  Clearly and completely outline your interest in the particular clinical category and how you demonstrate the qualifications necessary for the Resource Nurse position for that clinical category.  Please describe your strengths and abilities to meet all the criteria for the particular resource nurse category you are seeking. 

In addition, please complete the Self-Evaluation Form and include it in your completed application packet.
