PROCEDURE FOR TENCKHOFF CATHETER TUBING CHANGES

PURPOSE:  To provide directions for change of the peritoneal catheter transfers set.

KEY POINT: The PD transfer tubing is changed only if the tubing is cracked, leaking or contaminated. Notify physician of problem and establish need for antibiotics.

SUPPORTIVE DATA: Peritonitis is the primary complication of peritoneal dialysis. Meticulous care of the Tenckhoff catheter is necessary to reduce the introduction of contaminants into the peritoneal cavity.

EQUIPMENT:

  1. Peritoneal Dialysis Set change tray (Abbott #4185)
  2. Minicap Extended Life PD Transfer Set with twist cap (Baxter 5c44)

NURSING ACTION:

PROCEDURE STEPS:

  1. Gather supplies.
  2. Open PD Dialysis Set change tray.
  3. Arrange drape over lap to protect clothing.
  4. Pour Providine Iodine solution into sterile basin.
  5. Place Titanium adaptor into solution and soak for 15 minutes.
  6. When soak is completed, turn off room air, heater, and close the door.
  7. Place mask on self, patient and anyone else in room.
  8. Apply clamp (from tray) on Tenckhoff catheter (between patient and adaptor).  Open Baxter transfer set.
  9. Don sterile gloves.
  10. Using gauze pads grasp the Tenckhoff catheter in one hand, and transfer set in the other and disconnect the two.
  11. Discard the transfer set.
  12. Apply new transfer tubing, connecting white port to the titanium adapter with a twisting motion to secure.
  13. Maintain sterility of the titanium adaptor at all times. If the adaptor becomes contaminated, repeat Providine Iodine soak and notify physician.
  14. Document tubing change on PD flow sheet.
