SUBJECT: ASSIGNMENT AND MAINTENANCE OF COMPUTER SYSTEM ID AND PASSWORDS

1. New Employees.

    1. Before using the hospital's computer system, new employees will be oriented to the computer workstations and department policy regarding their use.
    2. Each employee eligible to use the Hospital Information System will be assigned an individual ID and Password after their orientation.
    3. These unique identifiers will represent the employee's legal signature when using the system and should not be shared with another person. Neither should an employee attempt to use another person's ID and Password to gain access. Misuse of IDs and Passwords may result in disciplinary action and suspension of access privileges.
    4. In order to receive user privileges, each employee must complete an Information Systems "Request For Access" form and sign the Security and Confidentiality Statement. This information will be routed to the IT security office, the employee entered into the system, and the Confidentiality Statement filed in the employee's personnel file. (Forms in Admin Guide 3.1.15.)

2. Transfers, Leave of Absence, long-term Disabilities/Terminations.

    1. The Nursing Business Office will generate a biweekly list of employees who have terminated, transferred, or are going on LOA or LTD.
    2. This list will include name, social security number, unit, and effective date. The Nursing Business Office will remove or reassign access to those named in the report according to the employee's status.

3. Forgotten or Nonfunctional Passwords.

    1. If an employee cannot remember their password or discovers that it will not allow access to the system, the employee should call  or email the Computing Services Help Desk.
    2. When proper identification is given by the caller, the password field will be reset to accept a new password. The employee is then responsible to enter a new password into the system and remember it.
