The Care Delivery Facilitator (CDF) provides clinical and managerial support to the patient care teams on designated units. The CDF functions as a clinical expert, teacher, researcher, consultant and care provider on designated units.  The Clinical Services Managers (CSM) assigns CDFs.

In addition the general duties and responsibilities outlined in the CDF job description, the 2A CDF is responsible for the following:

1. Reviews staffing each shift utilizing staffing matrix
2. Completes staffing variance form when staffing above the matrix
3. Validates accuracy and completion of staffing and census for Staffing Office for their shift
4. Makes patient care assignment, breaks, and unit cleaning assignment
5. Assists with staffing, staffing changes and call-ins
6. Communicates staffing changes promptly to the Staffing Office or ADON
7. Documents all staffing changes, pulls, on-call and hospital cancellations on the correct change sheet, Tour of Duty, or Schedule of Staffing (SOS) Sheet.
8. Fax corrected SOS Sheets to Staffing Office by 1000 daily
9. Ensures RNs complete Quadramed by appropriate time
10. Makes appropriate bed assignments
11. Ensures emergency equipment is completed for 2A each shift
12. Precepts unit employees on orientation
13. Validates orientation and annual competencies for all unit employees
14. Monitors MD order and documentation of all patient physical restraints each shift
15. Communicates patient/family problems or complaints to CSM or Assistant Director of Nursing
16. Rounds on patients each shift and prn
17. Rounds with physicians when possible
18. Provides shift report for on-coming CDF
19. Attends staff shift report when possible
20. Monitors all housekeeping on unit and ensures corrective action is taken as indicated
21. Assesses all discharge rooms after cleaning by housekeeping when possible
22. Ensure staff completes all unit shift assignments
23. Assists staff with patient care as needed
24. Assists staff with complex patient care issues
25. Assures staff utilizes down time productively
26. Assists with interim evaluations on 2A staff as requested by CSM
27. Submits variance reports to CSM as indicated
28. Informs CSM of patient/unit problems or potential problems
29. Ensure staff gets assigned breaks
30. Completes pyxis discrepancy reports each shift
31. Assures narcotic counts Monday and Thursday A.M. on all units
32. Communicates with College of Nursing instructors when nursing students assigned to 2A
