SUBJECT: SECTION J, WEIGHTS
  1. Daily weights are obtained at 0600 unless ordered otherwise.
  2. Weights are completed by a UAP unless patient’s condition warrants a RN.
  3. New patients are weighed upon admission, if patient condition allows, and then according to physicians or RN orders.
  4. Pre-op patient weights are completed and recorded on 24 hour flowsheet and pre-op check list sheet.
  5. If patient is not weighed at designated time, the reason is documented in medical records.  This is also communicated in shift report.
  6. All weights are obtained in Kilograms.  Weights that change by more than one Kilogram from previous date’s weight are reported to the RN.
  7. TPN patient weigh daily.
  8. Tube feed patients weigh every other day.
  9. Record as directed in 24 hr flow sheet and 2A weight book at desk.
