SUBJECT: ANEURYSM PRECAUTIONS

In order to provide safe nursing and the appropriate environment for the patient suffering a hemorrhagic stroke related to a cerebral aneurysm, the following precautions are initiated upon admission by physician or nursing order:

  1. Provide quiet, dark, private room
  2. Place "Aneurysm Precautions" sign on patient's door
  3. Limit visitors
  4. Perform and record vital signs with neuro checks every four hours and as indicated, reporting deviations from baseline to the physician (NO RECTAL TEMPS)
  5. Elevate head of bed 10-45 degrees or as ordered
  6. Initiate "Seizure Precautions"
  7. Monitor fluid balance
  8. Refrain from discussing illness/impending surgery with patient (refer patient's information requests to attending physician)
  9. Reduce environmental stimuli (limit TV, phone, caffeine, room paging, loud talk and unnecessary activity in the room)
  10. Perform all hygiene tasks for patient
  11. Deliver medications for pain, sedation and vasospasms promptly
  12. Report undue patient anxiety to the physician
