1. Each patient has a complete physical assessment a minimum of every 12 hours by an RN. Reassessments are completed as indicated in the patient’s plan of care and when the patient’s condition warrants.
2. Patients admitted post-operatively or that are emergency admissions will have documented assessment every 4 hours for the first 24 hours.
3. Assessments are completed when the patient's assigned nurse changes.  Patients transferred from ICU, PACU, ED or other units will have a complete biophysical assessment within one hour of arrival to 2A.
4. Patients admitted to 2A from clinics, admission, or direct admits will have a complete biophysical assessment within two hours of arrival on unit.
