TITLE: GUIDELINE FOR USE OF PYXIS ACTIVITY VERIFICATION LOG
REMINDER: Narcotic Counts are on Monday and
Thursday.
INSTRUCTIONS:
- Place unit name at the top of log.
- Place the date of the week in space provided.
- Place the date of the month beside the day of the
week.
- At the end of each shift, any nurse administering
drugs from the pyxis must verify the activity report for their shift and
sign the PYXIS Activities Verification Log.
a.
Run “Report” , “Activities” , “By User” then type part of your last
name
b.
Select your name from the list
c.
Push the “Next “ button to highlight Class II and “Print Report”
d.
Push the “Next” button to highlight Class III,IV, V and “Print
Report”
e.
Check activities listed for accuracy.
f.
Sign the Verification log indicating Title and Shift (in the S
column)
g.
Discard the activities listing in the unit trash or turn it in to
your Clinical Services Manager. Do not remove from the hospital.
- At the end of the shift, the CDF or designee will
run a 12-hour discrepancy check.
- Each discrepancy will be resolved with two nurses
present; each nurse should sign in the appropriate place for that shift.
- The CDF or designee for off-going and on-coming
shifts will sign in the Discrepancy Check column
- The CDF will also run a report of all users for
Class II, III, IV, and V drugs and review for their shift.
- Narcotic counts are completed each Monday and
Thursday morning by off-going and on-coming RNs
- Each patient care unit is responsible for monitoring
compliance with this policy
- Pharmacy will pick up the completed forms every
Monday morning.
- Blank forms will be provided weekly by Pharmacy.
