Change from HRAC to UAMS IRB

In the consent form replace Human Research Advisory Committee (HRAC) with University of Arkansas for Medical Sciences (UAMS) Institutional Review Board (IRB). This change should be in all consent forms submitted with NEW PROTOCOLS or in MODIFICATIONS to consent forms submitted during the study.
