Adverse Event Reporting
When reporting local adverse events, including death, the PI must provide an assessment in narrative form of whether the observed adverse event is related to the protocol.  Sufficient detail must be provided to assist IRB Committee evaluation of the relationship between the adverse event and experimental parameters in the protocol.
