Any domestic applicant must submit the following documents:

Official transcripts from each university attended even if listed as transfer credit on another transcript.

For all applicants, official test results of the GRE (MAT for Nursing Science only) score must be sent directly to UAMS by the Educational Testing Service. A photocopy of the scores sent to the student, who in turn forwards the scores to UAMS is not sufficient. No action will be taken on an application containing a photocopied test score. There is no exception to this policy.

Deans Distinguished Scholarship (only Ph.D. applicants can apply)

Speech Pathology Application for Assistantship (only Master of Science in Communicative Disorders/Speech Pathology applicants)

Letters of recommendation (check program requirements in on-line catalog for program requirements, if any)
