Fitness Center Registration

Who may join?

• UAMS faculty, staff and students
• VA employees
• Weight Control Clinic

VA membership and the weight control clinic fee is $30 a month with payments being made through bank or credit card draft.  All members without a UAMS badge must purchase a special fitness center ID badge at a cost of $5. 

Spouses/Companions – Employees and students are allowed to sponsor one additional person. All members must be 18 years of age. Please note that sponsoring one additional person will double the employee’s payroll deduction.

Instructions
Please make sure you fill in all applicable spaces, paying special attention to include your Social Security number, correct mailing address or mailbox #, email address, date of birth, etc. If you are registering a spouse or companion, be sure to print all four spouse/companion forms and include all information about that person.

Completed forms should be faxed to 526-7672, or mailed internally to mailbox #838. Forms can also be dropped off at the Fitness Center on the 8th floor of the College of Public Health building.
