Completed forms should be faxed to 526-7672, or mailed internally to mailbox #838. Forms can also be dropped off at the Fitness Center on the 8th floor of the College of Public Health building.

CANCELLATION 

You may cancel your membership by completing a cancellation form (PDF) and faxing it to 526-7672.  Please call and verify that your fax was received at 526-2222. 

We do not accept cancellation by phone or email.
