2009 Contract to
Get Healthy
The Contract to
Get Healthy will help hold you accountable for all of the New Year's
resolutions often made but forgotten after a few weeks. This program is
designed to encourage you to commit to an entire year of making healthier
choices in hopes of improving your level of wellness.
The rules are
simple:
(1) Sign a
"contract" promising to meet various monthly lifestyle changes for healthy
living.
(2) Choose
from the list of goals provided which one you want to complete each month. Four
(4) goals must come from the list of "Verifiable Wellness Actions".
(3) Goals
reported in advance will not be accepted.
(4) Goals may
be reported only for the current month and the previous month.
(5) You may
complete a goal only once.
(6) You are
required to complete only one goal each month.
Each participant
completing all 12 months will have their name entered into a drawing for a grand
prize. All participants completing 10 and 11 months will also be entered into a
drawing for prizes
Please keep in mind
that all winners will be required to provide documentation for their "Verifiable
Wellness Actions".
