Undergraduate Program

Evaluations

Sarita Prajapati, MD, MPH, Family Medicine Clerkship Director, in consultation with Kent McKelvey, MD, will manage the evaluation plan, with assistance from Michelle Haygood, Undergraduate Coordinator. The Course and Faculty/Preceptor evaluations will be done electronically by having the student log into the College of Medicine website during the last few days of the clerkship rotation. The curriculum evaluation remains the same as in prior years.

Students Complete

Course Evaluation: The purpose of this evaluation is to gauge the overall quality of the course. (This evaluation is one of the Dean’s office requirements.) This evaluation will be done using the College of Medicine website format.

Faculty/Resident Evaluation: The purpose of gathering this information is to evaluate the effectiveness of our clinical preceptors. This information will be shared quarterly with preceptors. This evaluation will be done using the College of Medicine website format.

Instructions: Students must use their social security number to access the evaluations and can only access the evaluation for the site in which they are working. The evaluations will be available at a specific time during the last week of the clerkship rotation. Go to: http://qmperception.uams.edu/q4/perception.dll to complete these evaluations.

Faculty Complete

Student Clinical Performance Evaluation: The purpose of this evaluation is to provide students feedback on their overall clinical performance in the course. This clinical evaluation will comprise 65% of the student’s grade for the course.

Mid-Course Assessment: The purpose of this evaluation is to identify students at risk of failing the course. Individual site directors will utilize this form as needed.
