Undergraduate Program
Patient Log Booklet
In accordance with the
requirements set forth by the College of Medicine, the
purpose of this component is to record each student-patient encounter in
the ambulatory and hospital setting. A patient log booklet will be carried
by the student during his/her clinical activities. Information should be
completed in the log for each patient encounter. You will need to bring your
log book to your coordinator or director at the end of your second week of
the rotation for them to view your log book and make sure you are on track
to see the required number of patients and
diagnoses. This will need to be
turned in at the end of the clerkship and signed by your Site Director.
For
each patient encounter, the following will need to be recorded:

Principal
Diagnosis
-
Date Seen
-
Preceptor
On the
front of the booklet, be sure and complete the “Student Name”, “Rotation”,
and “Dates”.
