SOSWorks Fax-back Referral Program

How can I get involved in the program?

SOSWorks is a free, proactive, centralized referral system that links patients and others to the state-sponsored, evidence-based cessation services in the state. The program targets all healthcare providers, i.e. primary care and specialists, dentists, hospitals, and third-party payers, but is also used by workplaces, individuals, chamber of commerce organizations, and others.

Through SOSWorks, tobacco users are linked to state tobacco cessation services in a three simple steps:

• Healthcare providers are prompted to ask all patients about tobacco use status and consent patients to SOSWorks via the HIPAA compliant SOSWorks Referral Form.

• The SOSWorks Form is faxed to SOSWorks by the healthcare provider via a toll-free fax number. Faxing the SOSWorks faxback form conforms to the usual office practice of faxing referrals for patients.

• SOSWorks calls the tobacco user at their convenience and discusses cessation options, provides motivational counseling, and refers them to cessation services or provides self-help and/or motivational and tailored materials.

• Providers are given feedback on all referrals.

• All referrals are followed for three months.
