After viewing the online presentations, access the quiz by going to http://qmp.uams.edu/qm/perception.dll, using the generic logon name of Pharmacist and generic password name of Pharmacist. 

When accessing the quiz, you will be prompted to give your name and social security number as identifiers.

To print a copy of the test, go to the File menu and choose Print. This must be done before submitting the exam electronically. After the submit button is selected, the option to print is not available.

Statements of credit will be mailed once the post test has been received and a grade of 70% or better has been achieved. Please complete the registration form and submit along with a copy of the post-test and evaluation form.
