Request for Letter of Reference

To the Applicant:

You must have two members of the science department (faculty member or instructor) write a letter of reference for you. Write your name legibly on the blank line below. Give this page of instructions to your evaluator. You should request a letter from your evaluator at least two weeks in advance of this deadline to allow him/her adequate time to prepare and mail his/her letter.

In your letter, please comment on the applicant's maturity and judgment, dependability and reliability, emotional stability, initiative and originality, personal appearance and presentation, abilities as a team member, communication skills, character and integrity, leadership potential, and potential as a medical student. Your letter should clearly state whether you highly recommend, recommend, recommend with reservation, or do not recommend this applicant as a participant in the summer science enrichment program for disadvantaged students at the University of Arkansas for Medical Sciences, College of Medicine. Answers to the following questions will provide useful information: (a) Applicant's science aptitude? (b) How long and in what capacity have you known the applicant? (c) What do you consider the applicant's talents or strengths? (d) How well has this applicant thought out his/her career plans?
