Solid Tissue and Tumor
    1.    Collected under non-sterile conditions
    2.    Transported in non-sterile, leaky, or dry container
    3.    Inadequate size sample
    4.    Received more than 24 hours after collection if not refrigerated,
           or 48 hours after collection if refrigerated
    5.    Unlabeled or mislabeled specimen
