Clinical Records

The following are links to DMS Clinical Records. Click on the document title you wish, then you can print out the blank forms you need.  There are HTML formatted files and duplicates in PDF formats. Depending upon how your Web Browser/Printer are set up, the HTML versions may not print as they appear on screen. For best results set all margins to 0.4" in the FILE>Page Setup menu.  The PDF formats will print exactly as they appear, but may take more time to download to your computer.  To open PDF files you will need Acrobat Reader.
