 Thank you for choosing our practice for the care of your family.  On behalf of the entire staff, we are delighted that you have placed your confidence in us.  We are committed to providing the best possible medical care. A doctor/patient relationship is a very special arrangement, one which requires the sharing of information.  That is why we have prepared this website.  Please take a moment to read it carefully.  It is designed to let you know how our office operates and what to expect when you visit.  

What is the Family Medical Center?

The Family Medical Center is affiliated with the University of Arkansas for Medical Sciences (UAMS), the Area Health Education Center of Northwest Arkansas (AHEC), Washington Regional Medical Center, and Northwest Medical Center.

Our Physicians specialize in Family Practice.  This means they are especially qualified to evaluate your total health needs, to provide you with personal medical care, and to refer you to appropriate specialist and community resources when the need arises.

Hours of Operation:

Monday - Friday 8:00 a.m. to 5:00 p.m
*except holidays.

Patient appointments are scheduled between 8:15 a.m. and 4:15 p.m.

The medical staff is unavailable between 12:00 p.m. and 1:00 p.m

Emergencies and After Hours:

If an emergency develops after office hours or on weekends or holidays, our physicians are on call at Washington Regional Medical Center at 713-1000 and Northwest Medical Center at 751-5711.  Please call or go to one of these facilities and ask for the physician on call for the Family Medical Center.

The Family Medical Center offers comprehensive medical services such as :

  • Office and hospital care

  • Preventive health care

  • Pediatric & adolescent physicals and immunizations

  • Adult wellness/preventative health care  exams

  • Physicals for schools, camp, sports, insurance, employment, and immigration

  • Prenatal care and delivery

  • Women's Health care

  • Nursing home care

  • Medical Testing and x-ray

  • Office surgery, (vasectomies, ingrown toenails, mole removal, circumcision, colposcopy, and other minor surgeries)

As a team of Health Care Professionals, we offer a comprehensive range of Patient Education and Health Counseling services to all of our patients through physician referral. 

These services include educational materials for any health issues and individual counseling for specific health problems, including Breastfeeding, Physical Fitness, Weight Management, Smoking Cessation, Hypertension, Hyperlipidemia, Diabetes, Asthma, CHF, COPD and Arthritis.

APPOINTMENTS

Office visits are arranged by appointment to ensure that you have time to discuss your concerns with your doctor.  Please call to make an appointment before coming in to the office.  Our phone number is 521-8260.

When your illness is urgent in nature, your doctor or another physician will see you within 24 hours.  Please call early in the morning, if possible, to arrange an appointment so that your illness can be treated.  By calling before coming to our office, you allow us to treat both you and other patients in a more timely manner.  

It would be of great benefit to our nursing staff and physicians if you would briefly describe to the receptionist your medical problems.  Please answer their questions as completely as possible.  

If more than one family member needs to be seen at one time, please make an appointment for each person.  If you must cancel your appointment, please call us in advance. This courtesy call will be appreciated by our staff and other patients.  

It is our Clinic Policy to charge a fee of $20.00 to every patient who does not give the Clinic prior notice of cancellation of an appointment.  We do have voice mail for after hours that a patient may use to leave word if they need to cancel.

Insurance Policy

As a courtesy to our patients, we file your insurance claims with the proper insurance company.  Please bring your current insurance card with you at the time of your visit.  Please understand that coverage varies significantly among insurance carriers; therefore, it is your responsibility to thoroughly understand the coverage and exceptions of your particular policy. 

Awareness of the unique provisions of your policy such as copay amounts, deductibles, and specific laboratory requirements, will aid in proper insurance filing and reimbursement.  Please notify our office staff of any changes in your insurance coverage.  If your insurance company requires referrals and/or preauthorization of services, please make sure this has been completed prior to your visit with the specialist.

PAYMENT FOR SERVICES

Family Medical Center expects payment at the time of service unless arrangements have been made prior to your appointment. 

Our office accepts payment by cash, check, money order, VISA, or Master Card.  Our fees reflect the usual and customary charges for this area.  However, our fees may vary slightly from those usual and customary fees cited by insurance companies, which often use outdated fee computations. 

Additional services, which may include some laboratory tests and pathology services such as pap smears and biopsies, will be billed separately by the facility performing those services

TELEPHONE CONSULTATIONS & PRESCRIPTION REFILLS 

If you have a question about your care, please call during regular office hours. Our receptionist will take some preliminary information from you to help our medical staff to better serve you. 

Please call about a refill approximately one week before you completely run out of medication.  We will make every effort to accommodate refill requests within 24 hours, if possible.  When you call, inform the operator of the name and dosage of your medication as shown on the bottle and the name of the pharmacy you prefer.  Some medications require medical supervision and will not be refilled without your being seen by your physician. 

If an appointment is necessary, refills will be made for only  enough medication to last until your next visit.  Please bring all medication bottles when you come for your appointment, and request your medication refill at this time.

CONFIDENTIALITY OF YOUR MEDICAL RECORDS 

All of your medical records and any information you give a staff member are confidential.  No information about you or your medical history will be released without your written authorization.
