1.)
|
Each of the 3 surveys you are being asked to complete requires approximately 5 minutes of your time.
To attend this workshop and receive continuing education credits, you must complete an Information Sheet and 2 Surveys (the Pre-Training Survey and the Post-Training Survey) at the time of training as well as any other continuing education evaluations as required by your professional group.
You must attend the training in its entirety.
You will receive a link to the Follow-Up Survey via email in about 6 months.
The Follow-Up Survey is an important part of the program evaluation.

2.)
|
The UAMS Office of Continuing Medical Education requires us to report your attendance in order to award CME credits.
If you require continuing education credit, we must report that you attended this training to the appropriate organization.
Additionally, the data from this training program will be periodically compiled and reported to the funder (Pfizer, Inc.) in aggregate, de-identified form.

Research

We would like to utilize your information as part of a research study.
An understanding of your responses to training will advance our knowledge of important issues in the dissemination of these practices, and may improve the quality of patient treatment and future trainings.
Publishing our findings allows the larger professional community to benefit from our experience.
Once data collection for the program is complete, the data will be de-identified, analyzed for research purposes, and submitted to a peer-reviewed scientific journal of publication.
If you would like copies of study findings once they are avaliable, we will be happy to provide them. Your identifying information will not be associated with any presentation of this data.
