How to Locate Your UAMS Badge Revision #
To ensure your UAMS Badge ID will grant you access to the door you're requesting, we will need the Badge ID Revision number.

The Revision # of your UAMS Badge ID can be located on the left edge of your picture on the UAMS Badge. Be sure to include ALL digits of the Revision # including any spaces in between digits.

If the Revision # does not exist, leave Badge Revision # field empty.

The image below should help you to identify and then enter the Badge's Revision number on the Door Request form.
