UAMS PAPR Respirators Approval Process

Since the Powered Air Purifying (PAPR) respirator is a tight-fitting face piece respirator, there is an approval process each wearer must complete before being fitted for the respirator. The steps below includes a medical questionnaire, training, testing and fitting of these respirators.


Step 1: Complete the Respirator Medical Evaluation Questionnaire.
  • Send the completed questionnaire to the Employee and Student Health Services by fax 501-296-1230 or send it to mail slot 530-8.
  • Please wait 3 to 4 business days after faxing or mailing the questionnaire to the Employee and Student Health Services to process the questionnaire. You will only be contacted if there are concerns regarding your answers on the questionnaire.
  • Resubmission of the Medical Evaluation Questionnaire is required:
    • Every 2 years if age is 45 years and older
    • Every 3 years if age is 35 - 44 years
    • Every 5 years if age is 34 years and younger
    • Every year if an individual has an existing or develops a medical condition that requires annual examinations.

Step 2: The Respirator Training must be completed before being allowed the wear a PAPR respirator.
  • Respiratory training must be completed before being allowed the wear a respirator.
  • Select and complete the appropriate PAPR Respirator Training and the post test.

Step 3: Hands on training is required for the use of any powered air purifying respirator.
  • You must make an appointment for hands on training by calling the Safety Compliance Manager, Sonja Hart at 501-296-1060
  • You must print the My Compass training certificate and bring it with you before you come to be fit tested.


Step 4: Annual online training and hands on training are required.
  • Annual online training and hands on training are required to monitor changes in facial structure and ability to wear the respirator correctly.
