In addition, this policy will enable the Physical Plant to account for, track and confirm that all Hot Work has been appropriately supervised to ensure proper precautions are taken to prevent fires. DEFINITIONS Hot Work: Any temporary operation involving open flames or producing heat and/or sparks. This includes, but is not limited to: brazing, cutting, grinding, soldering, torch applied roofing and welding. Designated Area: A safe area where hot work is authorized. Fire Watch: A person assigned to work with a welder, etc., normally outside an authorized area, to watch for fires resulting from hot work. Fire watchers will have fire extinguishing equipment readily available and be trained in its use. They will be familiar with the location of and the use of the nearest fire alarm or means to notify the Physical Plant Control Center. They should attempt to extinguish fires only when obviously within the capacity of their equipment; or otherwise sound the alarm. A fire watcher should have no other duties except to watch for signs of fire during the hot work operation and the 30-minutes following. Hot Work Permit: A special work order (permit), issued by the Physical Plant, which authorizes specific hot work at a specific location and time. Fire Safe: No threat of fire exists. RESPONSIBILITIES Each UAMS employee or contractor engaged in hot work is responsible for implementing controls that will eliminate or greatly reduce the potential fire hazards generated by their work. Such controls will help ensure the protection of other University employees, students, patients and visitors. Each supervisor, whose employee(s) engage in hot work, is responsible for ensuring that the guidelines in this Policy are implemented and hazards are controlled so as not to present an exposure to UAMS employees, students, patients and visitors. The supervisor is also responsible for ensuring the employee(s) they designate to perform hot work, use the necessary procedures and equipment to minimize that employee's own exposure to the hazards generated.  UAMS ADMINISTRATIVE GUIDE NUMBER: 11.1.11 DATE: 06/03/03 REVISION: PAGE: 2 of 5 The Occupational Health and Safety (OH&S) Department is responsible for providing professional expertise in training or counseling to supervisors and UAMS staff involved in the preplanning of job(s) that require hot work. The Physical Plant is responsible for issuing and tracking the permits used to authorize hot work. POLICY All employees from Physical Plant, Telecommunications, Information Systems, Media Services, other UAMS department personnel and contractors working for Construction and Contract Management will first submit a Physical Plant work order and get approval prior to performing any hot work operations. All hot work will conform to the minimum requirements of NFPA 51B, Fire Prevention During Welding, Cutting, and Other Hot Work. A permit may be requested by going to the Physical Plant Control Center and providing pertinent information on the work being performed. A set of safety precautions will be issued along with the hot work permit. In addition, hard copies of the Physical Plant Work Order (PP Form 2) (item #26953175) may be completed prior to reporting to the Control Center to expedite the process. Hard copies may be obtained from the Physical Plant Material Management warehouse or ordered from the UAMS Stockroom Catalog. SUMMARY OF REQUIREMENTS Permits: A Hot Work Permit will be obtained from the Physical Plant Control Center prior to performing hot work in the field. Permits are not required for work in approved permanent shop areas. Equipment: All equipment used for hot work will be chosen for safe application to the work and will be properly installed. Proper shielding and eye protection to prevent exposure to personnel from hot work hazards will be provided and used. Training: Workers designated to operate welding, soldering, cutting and other hot work equipment will be trained in the use of the equipment. Additionally, workers who operate welding equipment will be properly instructed and qualified to operate the equipment. Fire Prevention: Precautions for fire prevention in areas where hot work is being done will include isolating hot work activities, removing hazards from the vicinity and providing a fire watch. Ventilation: The need for exhaust ventilation will be evaluated case by case during the permit process. PROCEDURES Hot work will not be conducted in any area other than those specifically intended and designed for conducting said work unless the area has been inspected and a Hot Work Permit issued to the person(s) who will be doing the actual work. The individual taking out the permit must personally examine the location where the work is to be done and ensure that all safety precautions have been taken. These specific fire safety requirements are outlined on an attachment to the Hot Work Permit.  UAMS ADMINISTRATIVE GUIDE NUMBER: 11.1.11 DATE: 06/03/03 REVISION: PAGE: 3 of 5 The permit will be issued to one person responsible for work to be accomplished. Renewal requests will be made on a case by case basis Only approved apparatus such as torches, regulators, pressure reducing valves, acetylene generators, machines, manifolds, cables and hoses in good repair will be used. Manufacture's procedures/recommendations will be followed with respect to the sequence of operations for lighting or rendering equipment safe. Only acetylene tanks that have been in the vertical position for a minimum of 2 hours will be used. Torches will be pointed away from people and combustible materials when lighting. Torches will be lit with a friction lighter or stationary pilot flame. Torches will not be lit from hot metal. When equipment is not going to be used for periods of 30-minutes or more or when left unattended the equipment will be shut off. Only people who are trained and proficient will conduct hot work. Trainees may perform work only when their trainer or supervisor is in the immediate area. All personnel must be suitably protected against hazards generated by the work, i.e. heat, sparks, fumes, welding rays, etc. This may include, but not limited to, the use of personal protective equipment, shields, screens, or local exhaust ventilation. Permits will be issued by the Physical Plant Control Center only after providing the necessary instructions to ensure the work site is fire safe prior to any hot work. The Hot Work Permit is only good for the date(s) and time specified on the permit. Permits will generally be issued for a 24-hour period only. However, in areas where the work will be ongoing for an extended period, permits can be issued for periods not to exceed 72-hours as long as the work or situational factors do not change. Contractor permits may be issued for the duration of a contract or specific job from the applicable individual, as stated in 1.a. above. Periodic inspections may be made. This requirement applies to all contract work, no matter what department the work is performed in. The Hot Work Permit must be displayed at the job site, preferably attached to or near the welding or cutting equipment. Permits issued to shops will be displayed in a prominent place at the approved work area. In sprinkled areas, sprinkler protection must be fully operational while hot work is being performed and the proper fire extinguishing equipment must be readily available. If hot work is to be done within 3 feet of automatic sprinkler heads, noncombustible sheet material or damp cloth guards will be used to temporarily shield the individual heads. The supervisor responsible for conducting the pre-work inspection will ensure compliance prior to allowing the work to proceed. Hot work will not be conducted in buildings with installed sprinkler systems when the system is inoperable, unless other means of fire detection, reporting and extinguishing have been approved by the Fire Marshal and OH&S has been notified and consent to the work.  UAMS ADMINISTRATIVE GUIDE NUMBER: 11.1.11 DATE: 06/03/03 REVISION: PAGE: 4 of 5 Smoke detectors in the immediate area may be bagged per the instructions of the OH&S representative in order to prevent contamination of the head and reduce nuisance alarms. This will also leave as much of the fire detection system as intact as possible to detect abnormal conditions in other parts of a facility. When a detector is bagged the personnel performing the work necessitating the bagging will remove the bag immediately after finishing the work. If work will extend throughout an entire day or following days the bags will be removed any time the personnel conducting the work will be gone for 30 or more minutes. When it is necessary to conduct hot work in a confined space, permits will not be issued unless OH&S has approved entry into the area. When working in a confined space the equipment will be positioned outside the confined space in such a manner that it will not interfere with egress. Where possible, all combustible materials located within 35-feet of the operation (example: items that still contain or have contained explosive hydrogen gas, etc.) will be removed. Hot work will never be conducted within 50-feet of flammable liquids (flashpoint below 100 degrees Fahrenheit (F)). Whenever there are cracks or other floor openings that can not be closed or covered within 35-feet of the hot work, precautions may be directed by OH&S dependent on the situation to remove or otherwise protect combustible materials on the floor below that may be exposed to sparks. The same precautions will be observed with regard to cracks or openings in walls, open doorways, and open or broke windows. Combustible floors and other interior finishes will be kept wet or protected by fire resistant shielding. Where floors have been wet down or shielding put down, rigid control will be exercised over the grounding of equipment. In addition, personnel will be protected from possible shock. Caution must be exercised against slips and falls on any wet surfaces. A fully charged and operable ABC fire extinguisher will be kept in the immediate work area at all times while hot work is in progress. Note: If fire extinguishers are located inside of buildings or at distances greater than 15-feet or where obstacles are in the direct path of access, then the extinguisher will not be considered immediately accessible. Fire watcher(s) procedures will be implemented whenever hot work is being conducted within 35 ft of combustible materials, regardless of the protection provided. A qualified person proficient in the operation of available fire extinguishing equipment and knowledgeable of emergency reporting procedures will observe the hot work area. Their responsibility is to detect and prevent the spread of fire produced by the hot work. An additional fire watcher may be required to observe areas that are hidden from the view of a single fire watcher (other side of partition, walls, ceilings, etc). A fire watch will be maintained for at least 30-minutes following the completion of the hot work to detect and extinguish possible smoldering fires. Following the completion of hot work, the permit must be closed-out with the date and time of the final inspection. When hot work is to be conducted in outside areas, the work should be moved to a fire safe area. If this is not possible, the area will be made fire safe by removing or protecting the combustibles from ignition sources. Appropriate fire fighting equipment is not readily available. Combustible or flammable materials are within 35-feet and cannot be moved or protected. Floor and wall openings cannot be covered. Flammable vapors or gases are present. Cutting or welding on pipes or other metals can conduct enough heat to ignite nearby combustibles materials. Any condition that could result in undue hazards by performing the work.
